Prior to your appointment

  • Please confirm with your health insurance carrier that your insurance is active
  • Please be informed of copay, co-insurance or deductible that you may have

We will need the following for your first appointment

  • Picture ID
  • Insurance card
  • Electronic referral if your plans requires you to have a referral
  • List of your current medications with doses and schedule
  • Pharmacy information
  • Name of referring provider
  • Any laboratory results or imaging studies reports or films that are relevant to your consultation

Follow up appointment

  • Insurance Card
  • Electronic referral if your plans requires you to have a referral
  • List of your current medications with doses and schedule
  • Please remember to have your labs done one week prior to your follow-up appointment if Labs have been requested by your provider

Communication

  • All non-urgent messages will be answered within 48 hrs.
  • Appointment requests can be made by calling the office or by requesting appointment through website. Please note this does not confirm your appointment until we can respond.
  • Medication renewal requests are processed within 48 hrs.
  • Please note, you do not need to wait until your last dose of medication to request the renewal of your prescription.
  • In case of medical emergency please call 911 or go to the nearest emergency room. Other urgent medical issues should be communicated to Dr. Lalani through hisĀ front desk staff.Ā  If you are not able to reach us directly, please leave a voicemail for the appropriate staff member.
