  • Please be informed of copay, co-insurance or deductible that you may have

We will need the following for your first appointment

  • Picture ID
  • Insurance card
  • Electronic referral if your plans requires you to have a referral
  • List of your current medications with doses and schedule
  • Pharmacy information
  • Name of referring provider
  • Any laboratory results or imaging studies reports or films that are relevant to your consultation

Follow up appointment

  • Insurance Card
  • Electronic referral if your plans requires you to have a referral
  • List of your current medications with doses and schedule
  • Please remember to have your labs done one week prior to your follow-up appointment if Labs have been requested by your provider

Communication

  • All non-urgent messages will be answered within 48 hrs.
  • Appointment requests can be made by calling the office or by requesting appointment through website. Please note this does not confirm your appointment until we can respond.
  • Medication renewal requests are processed within 48 hrs.
  • Please note, you do not need to wait until your last dose of medication to request the renewal of your prescription.
  • In case of medical emergency please call 911 or go to the nearest emergency room. Other urgent medical issues should be communicated to Dr. Lalani through his¬†front desk staff.¬† If you are not able to reach us directly, please leave a voicemail for the appropriate staff member.
